Desperate Help Pleaseeeeeee!!!?

Ok so where do i start, my micro sd usb 2.0, isnt reading anything, it isnt even alerting my laptop about anything, as it should do. I use this usb for my nds rom, which the memory card is also not working now, when i open up the r4 card, it says Loading, so i tried it in my sisters r4 device, ( my memory card ) it came up with SD/TF card error, everything was fine until i transferred a nds file onto it. I’d just like to know what the problem is, and how i can fix it, ive herd of formatting it, i dont know how to format it, please explain. best answer gets 10 points.

if i bought a new r4 and usb, and put the same file on again, would this happen again?
